1. Challan paid at State Bank of Hyderabad (SBH), Gruhakalpa Branch, Opp: Gandhi
Bhavan, Nampally, Hyderabad.
2. Application form for PCC(1 Copy).
3. Personal Particular Form.
4. Clear Xerox of Passport all pages including blank pages, ensuring the print of
Dotted number of the passport, bring the original passport for verification.
5. Residential proof of present address and previous places of stay if any in Cyberabad
limits viz., (any two of the following).
- BSNL telephone bill
- Electricity Bill
- Gas connection paper
- Lease deed in case of rented
house
- Household supply card ( ration card )
- Voter identity card
- Employer letter confirming residential address

Statement of bank account from nationalized banks only
- Residential Certificate
from MRO
6. Purpose of PCC (Letter of Proof).
- Consulate letter
- Visa copy
- Job offer letters from companies
7. Three {3} Photographs should be pasted on the application form and two {2} should be attached separately.
8. No objection certificate from the Head of the Department/Employer, if the applicant
is an employee.
9. Study certificate (if the applicant is a student).
10. The applicant should also mention clearly the periods of stay in Cyberabad with
dates.
